File of Life

Why should you have a File of Life?

PGFPD believes everyone deserves quick, quality medical care. The File of Life is designed to provide critical information to first responders.

Medical emergencies can happen anytime. When they do, there might be confusion, panic, or urgency. Family or friends might not know critical medical information that our crews will need in order to properly assess, manage, and treat your medical condition.
When we arrive at your residence for a medical aid request, we will need to know the following information: what is your medical history, any allergies, and what medications do you take.

Other important information we need is who would you like us to contact in case of an emergency.

The File of Life puts these answers at the fingertips of our crews and allows them to immediately begin the best possible treatment, notify loved ones, and pass vital information on to awaiting physicians at the emergency room.

To obtain a File of Life, stop by our main headquarter station in Platteville or request one be mailed to your house. If you need to update your current one, we would be happy to send you a new one as well.

The File of Life is free of charge courtesy of the Assistance League of Greeley.
